<p>Many patients achieve pain relief through treatment with a car Proctor, and for those who need supplemental therapy than simply the rehab and the adjustments in the office, a manipulation of anesthesia may do the trick. This is utilized when patients are not responding to normal treatment protocols, or could be having pain due to their degenerative condition that is just not getting relief.</p>
<p>The procedure that is specific for breaking up scar tissue along with adhesions in the neck, back, shoulder, a combination of short lever manipulations, along with some passive range of motion are utilized in the manipulation under anesthesia treatment.</p>
<p>Differing levels of anesthesia</p>
<p>Three different types of anesthesia may be used for the treatment, the 1st, is the least invasive and involves manipulating the tissues after local anesthesia has been injected into the surrounding tissue. During this type of therapy individual remains alert and awake, but the region that is manipulated is non-so severe pain is not felt dying the procedure. This is called local or local anesthesia because the numbing medicine remains in the specific region of the procedure.</p>
<p>The second method of anesthesia used during this procedure is mild sedation. The patient is sedated but awake, this state of sedation causes the patient to be relaxed and to feel less discomfort, if any, during the procedure. This method of treatment may be utilized with the aforementioned local anesthetic method. This method is slightly more invasive than the first as the patient would not be allowed to drive themselves home after the treatment and so would need to bring a friend or family to help them after the treatment.</p>
<p>The next method of treatment involves general anesthesia where the patient is put completely \&#8221;under\&#8221; or unconscious. This method is used for the more advanced levels of treatment where the patient would not be able to tolerate the pain associated with the treatment due to their advanced condition. Only licensed specialists perform this treatment in a hospital or surgery center. This type of treatment requires special practice, training, and certification.</p>
<p>Choreographed Teamwork</p>
<p>Rather than just the one person performing the treatment as you may be accustomed to with normal chiropractic adjustment, manipulation under anesthesia requires a team of three. There is an anesthesiologist, a main or \&#8221;prime\&#8221; physician/surgeon/chiropractor specially trained in this procedure and finally, an assistant physician/chiropractor that is also trained in this specialty procedure. This method of treatment has been practiced for about sixty years and is a recognized treatment by the American Medical Association.</p>
<p>Candidates for the manipulation would be individuals who have not responded to traditional chiropractic therapy due to scarred tissue surrounding the spine or shoulder secondary to injury.</p>
<p>This therapy can be utilized in conjunction with surgery or physical therapy. Typically this would be tried prior to surgery and candidates for the manipulation would typically undergo 6 to 8 weeks of conservative treatment. As with other therapies, success will vary from person to person but many have found relief through manipulation under anesthesia.</p>

<p>Do you experience either constant or intermittent back pain? You are not alone! A recent study on back pain showed that between 8 and 33 percent of the population is tormented by chronic lower back pain.</p>
<p>Chronic low back pain is classified as pain that carries on for 4 weeks or more. According to the National Institute of Neurological Disorders and Stroke, back pain is the second most widespread neurological disorder in the United States, second only to headaches. Back pain is the most common reason for job-related disability and missing work. It can disrupt your daily routine, your work, activities, and recreation.</p>
<p>The National Institute of Health gives a variety of different reasons behind chronic back pain, including:</p>
<p>-	Straining or overstretching the back, creating extra pressure on the nerve endings</p>
<p>-	Injury or trauma to the spine</p>
<p>-	Degenerative conditions, like arthritis, osteoporosis, and other bone diseases</p>
<p>-	Viral infections</p>
<p>-	Inflammation or excessive strain to the joints, muscles, and ligaments, as a result of overstretching</p>
<p>-	Degenerative conditions, such as arthritis, osteoporosis, and other bone diseases</p>
<p>-	Other health concerns, such as smoking, obesity, poor posture or sleeping position, stress, etc.</p>
<p>-	Aging, where the spinal discs lose fluid and flexibility, resulting in a reduction in the cushioning of the vertebrae</p>
<p>In our consumer-driven society, there are an infinite number of different treatments available to lessen chronic lower back pain. Americans spend more than $50 billion per year on low back pain and their treatments. Now the trick is to observe which treatment is the best, most efficient, and will have the longest lasting results!</p>
<p>A 2008 study published in The Journal of Alternative and Complementary Medicine compared classic medical practices with chiropractic care for treatment of chronic back pain. The traditional medical procedures used standard pharmaceutical therapy (i.e. anti-inflammatory drugs) and joint and soft-tissue injections to combat the low back pain. As for the chiropractic treatment, a widely-recognized treatment program for lower back pain was utilized.</p>
<p>An evaluation of the treatment programs was completed after 8 weeks of care, in which the research shows that the chiropractic approach is 457 percent more efficient than typical medical treatments for chronic low back pain. Notice, though, that this does not overlook the place that the medical route has in healthcare; however, this and other studies on lower back pain treatments conclude that chiropractic care is a considerably better and more successful choice. Doctors of chiropractic are trained to find the cause of injury and make a proper diagnosis in each patient, as well as build the best possible care plan for each patient.</p>

<p>Lower back pain causes such as nerve irritation, lumbar radiculopathy, bony encroachment, lumbar strain, and joints and bone conditions are some of the reasons you feel pain in your lower back. All these lower back pain causes will be discussed in this article.</p>
<p>1. Irritation in the nerves</p>
<p>Being one of lower back pain causes, nerve irritation can cause the sufferer too much pain because any disease along the path of nerves can irritate the lumbar spine. This can occur anywhere from the spinal cord roots to the skin surface. This condition is often associated with different back pain diseases such as lumbar disc disease, bony encroachment, and nerve inflammation.</p>
<p>2. Lumbar Strain</p>
<p>Lumbar strain is one of the known lower back pain causes suffered by many low back pain patients. This condition often occurs after an injury caused by stretching occurred in the tendons and ligaments in the lower back muscle. This occurrence causes varying levels of microscopic degenerations in the tissues affected by the injury, often through overuse improper use, or trauma. Lumbar strain is considered acute if the patient has experienced pain for only days or weeks while it is considered chronic if the pain is experienced for over three months.</p>
<p>3. Bony Encroachment</p>
<p>Bony encroachment, one of the common lower back pain causes, is the result of any growth or movement of the lumbar spine vertebrae that limits the encroachment or space for the adjacent nerves and spinal cord. Bony encroachment is caused by foraminal narrowing, spondylolisthesis, and spinal stenosis. These conditions often lead to sciatica pain that runs down the lower portion of the body. Spinal stenosis causes pains in the lower extremity, which can be worse with walking activities and can be better by resting. Treatment of these conditions can vary depending on how severe the condition and can range from resting to surgical decompression done by removing the bone that compresses nervous tissue.</p>
<p>4. Lumbar Radiculopathy</p>
<p>Lumbar radiculopathy is one type of irritation in the nerves, often caused by damaged discs located along the vertebrae. This can be due to wear and tear of the discs\&#8217; outer ring, traumatic injury, or combination of the two. As a result, the central portion may be damaged, thereby causing sciatica pain characterized by extreme pain in the legs.</p>
<p>5. Joint and Bone Conditions</p>
<p>These conditions are also known lower back pain causes. Joint and bone conditions are classified into congenital injury or the injury present since birth, degenerative injury caused by the wear and tear of the muscles, tissues, and bones, or injuries resulting from the inflamed joints also known as arthritis. Congenital injuries are further classified into scoliosis and spina bifida. Spondylosis is one of the known degenerative injuries, characterized by the narrowed space between the vertebrae. Arthritis and fractures are also one of the known joint and bone conditions.</p>

With these devices, surgeons can get a well-rounded view of the patient\&#8217;s injury for planning and preparing surgical procedures.</p>
<p>Before this pioneering medical device was created, surgeons relied solely on CT scans and MRI\&#8217;s. With the new 3D model being used as a diagnostic tool, they can provide valuable information in diagnosing certain injuries and diseases, since they give the surgeon an up-close and in-depth realistic view of the area of concern.</p>
<p>3-D models are made by medical stereolithographers who are experts in this field. With the use of a MRI scan or CT scan, the stereolithographer can duplicate a three-dimensional model that is exactly like the anatomy of the patient. The accuracy of the model is within 1/35th of a millimeter. This allows the surgeon to hold the model and inspect the area of concern closely, which otherwise can only be evaluated with imaging results. These three-dimensional models can be made for any skeletal part including the cranium and cervical, thoracic, and lumbar spine.</p>
<p>Not only does this new and innovative medical tool allow the Spine surgeon to analysis and diagnosis orthopedic conditions, it also is valuable in the practicing and review of precisely what the orthopedic surgery will involve. doctors can do a dry run of the procedure before the actual operation, which can reduce the margin of error extremely and reduce the amount of time the operation would normally take.</p>
<p>This provides the patient with much lower costs of operating room (in some cases the 3-D models can save over $10,000 in surgical costs) and surgical team fees as well as anesthesiologist and surgeon fees. Patients are also under anesthesia for a shorter amount of time, helping to reduce the risks of complications.</p>

<p>I have covered the rationale and aims of performing routine exercises for the spinal joints in a previous article, now I will move on to the performance of the spinal exercises themselves. Patients should follow the exercise instructions and perform the exercises smoothly with even timing through the range, holding for a short period at the end of the joint ranges. <p>Leg flexion to the chest In supine hold on to your knee and pull your thigh up to your chest, keeping it at the full extent for a few seconds, with the other leg remaining flat down. The lumbar spinal joints, ligaments and muscles, hip and sacroiliac joints are mobilised during this manoeuvre.</p>
<p>Bilateral knees to chest stretch Lying flat on the back, bend your knees and pull on your shins, pulling your thighs up to the trunk. This is less stretching for the sacroiliac and hip but gives a stronger stretch to the low back structures including the ligaments, muscles and joints.</p>
<p>The Pose of a Child Kneel on the floor and allow the trunk to curl forward to lie on the fronts of the thighs with the back stretching out into flexion. This flexes the whole of the spine because the bodyweight increases the force of the stretch.</p>
<p>Squatting down This movement involves a greater degree of force than previous ones and can be useful to counteract the effects of sitting for too long. Extension movements are often recommended to restore the lumbar curve after sitting too long but flexion can be just as helpful in relieving discomfort.</p>
<p>Squat right down until your thighs are against your calves, using a block under your heels if you need to maintain balance. Staying down in that position for half a minute, allowing the lumbar spine to flex out, can be performed at times or three times in a row with rests between.</p>
<p>Stretching out at the bottom of the movement, the back is kept in this position for half a minute or so at a time.</p>
<p>Lying on the front Sometimes the ability to extend the lumbar spine is restricted and then prone lying, lying on the front, is a useful starting exercise as even this can stress the joints when they are stiff. The back is more extended in this position that it appears on the surface.</p>
<p>Elbow supported prone lying A progression from lying on the front is to get the patient to support themselves up on their forearms to increase the extension stretch on the lumbar spine. The lumbar spine is placed in greater extension than prone in this position, stressing the tight structures and forcing them to give.</p>
<p>McKenzie Repeated Prone Extensions McKenzie technique is a form of manipulative treatment at affecting disc dysfunction and derangement. Lying on the front with the hands placed near shoulder level, the patient pushes until their arms are straight whilst leaving the pelvis down on the bed, involving a significant lumbar extension.</p>
<p>This exercise is known to be aggravating in certain patients as it forces the facet joints together so needs to be tested by a physiotherapist to ensure effectiveness.</p>
<p>Lumbar Rotations &#8221; Knee Rolling Lying on the back, the knees are bent and both knees are rolled to one side and then another, allowing the movement to go as far as it comfortably can. Although there is little rotation in the lumbar joints some of the spinal structures can be tight into rotation.</p>
<p>Lumbar Rotation Mobilisation The patient lies on their back and leaves their shoulders on the ground as they bend one leg up so the opposite hand can get hold of the knee and pull the leg over the body, stretching out the back. This stretches the facet joints and soft tissue structures strongly so other stretches might be more appropriate before progression is made to this exercise.</p>

<p>Colles&#8217; fractures, named after Abraham Colles who first described in 1814 the common fracture of the last inch of the radius and ulna near the wrist, is a very common consequence of a fall on the outstretched hand (FOOSH). Typical treatment is immobilisation in a plaster of Paris or similar material for five to six weeks to allow bony union, followed by a rehabilitation period of a month or more, a short period of which might involve a wrist brace for comfort during activity. Due to the functional importance of the hand, the period of immobilisation is kept to a minimum to prevent dysfunction of the hand and wrist.</p>
<p>Once the Plaster of Paris has been removed the physiotherapist will examine the wrist for appropriate healing by firmly palpating the area over the fracture, which should not show much more than mild tenderness. The hand should look a natural colour, have no tightness or swelling in the fingers and muscle wasting should not be severe. Movements of the wrist will be restricted in a few planes but should not be affected in all planes of motion, neither should there be severe pain on movement nor pain on all movements. If many problems are present the physiotherapist will take urgent steps to rehabilitate the patient.</p>
<p>Initial treatment is to instruct the patient in range of motion exercises to be performed every two hours. For many fractures this is all that is required as the movements are easily restored with a few days&#8217; exercises, concentrating on the end ranges of movement. The shoulder and elbow are checked to make sure they are not limited as they may have been injured in the initial incident or kept very still by the patient whilst in plaster. The pronation and supination movements of forearm rotation are functionally very important, and the physiotherapist checks wrist extension and flexion and finger and thumb movements.</p>
<p>Patients often report that the wrist feels at risk after the plaster has been removed and this may be due to the early removal of the plaster to prevent functional loss from immobilisation. A futura brace, a fabric support stiffened with a metal piece under the wrist, is applied with Velcro straps to give support during normal activities of daily living. The brace should be taken off during rests or light activity and for regular performance of the exercises. Too much further immobilisation at this stage could be harmful so patients should understand the limited use of the splint for comfort during activity.</p>
<p>If the ranges of motion do not improve as they should then the physiotherapist will consider using joint mobilisations to ease the movements. Accessory movements can be performed to the inferior radio-ulnar joint to help pronation and supination, and to the radiocarpal (wrist) and midcarpal joints, with the physiotherapist fixing one side of the joint as he or she moves the other side of the joint passively. This can be done gently or more vigorously at the end of range to push against the restrictions within the joint. Mobilisations can also be performed with the joint at the end of its available movement to give it the sliding and gliding movements it requires.</p>
<p>Returning steadily to normal use of the wrist and hand is the easiest and often the most successful way to regain forearm strength. In some cases more must be done to return the hand to normal if it is very weak or the person needs to return to a heavy manual job or has particular upper limb strength requirements for a sport or hobby. Instruction in practicing all the different hand movements against resistance can be accomplished in a hand class, where patients can use equipment designed to strengthen particular movements such as gripping, pulling, twisting, turning and to improve fine hand function.</p>
<p>If the hand is very painful, swollen and restricted in motion then treatment may be urgently directed to preventing a pain syndrome developing, once the fracture has been reviewed by a doctor to make sure healing has progressed as it should. Hot and cold contrast bathing for the hand can be useful for the pain and swelling, with massage and sensory work to reduce the hypersensitivity which can be troublesome. Patients need to be very clear that they need to work hard through the pain in these cases to regain a normal hand.</p>
